Issuing a Copy of Residence Certification
When your address has changed, you need to submit a document at City Office.
Period of Application

Within 14 days of moving to Minoh

- Residence cards for everyone in your household who moved
- My Number cards (individual number card) for everyone in your household who moved, or basic resident registration cards (if you have them)
- Moving Out Certificate from your previous place of residence
- A document showing the relationship between the head of household and other members of the household
- Your personal seal (or provide signature in-person)

When you need Proof of Personal Identification
As a registered citizen of Minoh City, you can receive a copy of your residence certification if needed to confirm your address.
